The Gait Center serves the entire Richmond metropolitan area, the state of Virginia and beyond. For your convenience, we request that you make an appointment by calling 804-523-2653. Our office is open Monday through Friday from 8:00 a.m. to 4:30 p.m. If you are unable to keep an appointment, please cancel as far in advance as possible so we might accommodate other patients. 1. Doctor's prescription
2. Primary Insurance Card
3. Secondary Insurance Card (if applicable)
4. Referral if required from Primary Care Physician
5. Optional - Patient Registration/History forms...only if completed in advance
